First Office Visit

Required items for your initial visit with Digestive Health Associates.  Please bring these items with you on the day of your appointment:

•   Current driver’s license
•   Current medical insurance card(s)
•   Copy of your insurance referral (if your insurance requires you to have one).
     If you are not sure, please check with your insurance company prior to your visit
•   List of all medications you are taking
•   Any blood test results (6 months or sooner)
•   Any CT scans, ultrasounds, or radiology reports
•   Your primary physician’s reports
•   Any other type of information that would assist the physician with your initial appointment.

New Payment Arrangement

Effective January 26, 2010 we are utilizing a payment solution that allows patients to use a credit card or a debit card for office visits. This eliminates the confusion of multiple bills and provides peace of mind about the accuracy of the payment process.

Please bring your insurance card as well as your payment card to your next office visit.
