If you are planning your first visit to Digestive Health Associates, we look forward to meeting you. To help prepare you, we have included all of the information you need about or practice on this page, such as a map and directions and our new patient form, which you can print and complete prior to your office visit.
Please make sure you bring the following items with you on the day of your appointment:
Current driver’s license
Current medical insurance card(s)
Copy of your insurance referral (if your insurance requires you to have one)
If you are not sure, please check with your insurance company prior to your visit
List of all medications you are taking
Any blood test results (6 months or sooner)
Any CT scans, ultrasounds, or radiology reports
Your primary physician’s reports
Any other type of information that would assist the physician with your initial appointment.

Insurance and Billing
Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own gastroenterologist.
